The Regional Coordination Centre (RCC) offers the following services to support patient navigation and access to service:

 

 

Diabetes Central Intake

Diabetes Central Intake

This program provides a central location to send referrals to diabetes education programs and diabetes specialist consults.  Referrals are triaged and sent to the appropriate Diabetes Education program and/or specialists.  Wait times are monitored to ensure people are being seen in a timely manner.

Orthopedic Central Intake

Orthopedic Central Intake

This program provides a central location for physicians or nurse practitioners to send referrals for consult with an orthopedic surgeon.  Referrals are reviewed for completeness and sent to the appropriate surgeon or triage and assessment clinic.

 

For fractures,  physicians or nurse practitioners, please call the Hospital "on-call" Orthopedic Surgeon for direction.

To improve access to hip and knee consults, referrals with moderate to severe Osteoarthritis will be routed from Orthopedic Central Intake to the Hip and Knee Orthopedic Assessment Clinics (OAC) based on specific criteria.  These clinics offer an assessment by an advanced practice physiotherapist in partnership with surgeons.

    

The OAC clinic locations for Waterloo Wellington are:

     Kitchener Waterloo: MSK Centre  

     Guelph: Wellington Ortho and Rehab

     Cambridge: Cambridge Memorial Hospital

     Mount Forest:  Claire Stewart Medical Clinic

     Palmerston:  Minto Rural Health Centre

 

To improve access to low back pain consults, referrals that meet the criteria for RAC-LB (Rapid Access Clinics-Low Back), formerly ISAEC (Inter-professional Spine Assessment and Education Clinics), will be directed to the most appropriate location in the Waterloo Wellington region.

Regional Self-Management Program

Self-Management Program

This program coordinates workshops for individuals with chronic diseases throughout the region.  The Self-Management Program also provides various training opportunities for health care professionals to develop greater skills and knowledge to enhance patient care.

Ophthalmology Central Intake

This program provides a central location for health care providers to send referrals to an opthalmologist for a cataract surgical consult. Referrals are reviewed for completeness and sent to the appropriate specialist. Wait times are monitored to ensure people are being seen in a timely manner.

 

For urgent referrals, please follow standard procedures or contact the Hospital "on-call" Ophthalmologist for direction.
